Patient Rights and Responsibilities

Your Rights as a Patient

As our patient, you have the right to:

  1. Be welcomed and treated with courtesy, respect, and confidentiality at all times.

  2. Receive care from a licensed physical therapist or under their direct supervision, with full transparency about who is providing your care.

  3. Be evaluated and treated without discrimination of any kind.

  4. Expect care in a safe, accessible, and compliant facility.

  5. Receive clear, timely explanations of your diagnosis, prognosis, treatment plan, and alternatives — including the risks and benefits of each option.

  6. Participate actively in your care decisions, including the right to decline or discontinue any part of your plan of care.

  7. Have your medical history, treatment records, and personal information protected in compliance with HIPAA and state privacy laws.

  8. Review and obtain a copy of our Notice of Privacy Practices.

  9. Be informed of your financial responsibilities prior to or at the time of service.

  10. Request and receive an itemized statement of services provided.

  11. Request access to interpreter services, if needed.

  12. Expect transparency regarding any professional or financial relationships that may influence your care.

Your Responsibilities as a Patient

As part of your care partnership, we ask that you:

  1. Provide complete and accurate medical, personal, and billing information.

  2. Participate in your treatment plan to the best of your ability — including attending scheduled appointments, following prescribed home exercises, and observing safety precautions.

  3. Communicate openly with your therapist about your progress, goals, and any barriers (such as health concerns, work, or personal commitments) that may affect your care.

  4. Demonstrate respectful and courteous behavior toward staff, other patients, and visitors, including maintaining appropriate personal hygiene and respecting the privacy of others.

  5. Notify your therapist or staff promptly if you have a concern, complaint, or suggestion about your care.

Good Faith Estimate Notice

You have the right to receive a Good Faith Estimate of expected charges for medical services.

What is a Good Faith Estimate?

Under the No Surprises Act, health care providers are required to give patients who are uninsured or not using insurance an estimate of the cost of care before services are provided.

A Good Faith Estimate outlines the total expected cost of your treatment — including physical therapy sessions, procedures, or related items.

Your Rights

  • You have the right to request a Good Faith Estimate for any non-emergency service.

  • You can ask for an estimate before scheduling or at the time of scheduling your appointment.

  • If your bill is $400 or more above the estimate, you have the right to dispute the charge.

  • Be sure to save a copy of your Good Faith Estimate for your records.

Dispute Resolution

If you receive a bill at least $400 more than your Good Faith Estimate, you may begin a dispute resolution process through the U.S. Department of Health & Human Services (HHS).
